To register for U9 AA SHERWOOD FALCONS, please provide the following information to amh_19@hotmail.com

Please register and pay before Friday, September 16th 2022 with the following information
 
- Player Name 
- Position (Skater or Goalie)
- Date of Birth
- Parent/Guardian Name
- Telephone Number
- Email Address

Phase 1 - Costs $30 per player (guaranteed 3 ice times) e-transfers can be sent to amh_19@hotmail.com
